To enrol your child in Kids Club Dalby, we require the following forms to be
filled in and returned to us at the interview.

Booking Form
Parent Information Letter
Parent Handbook to be read.
Any questions you may have regarding the above, please don't hesitate to bring them up in the interview.
****Change of Booking/Cancellations****
Any Changes of bookings are to be submitted 2 weeks prior to the date of change. We require a Change of Booking form to be filled in and given to a staff member. You can print the form, fill it out, scan it and then e-mail it if you like. Verbal changes will no longer be acceptable - Sorry.
